An Audit of Screening Ultrasonography in Patients after Surgery for Breast Cancer

Abstract

|
Purpose: This study was designed to perform an audit and to evaluate the performance of screening ultrasonography (US) in patients after surgery for breast.
Materials and Methods: Between January 2008 and December 2008, 5,343 breast sonographic examinations were performed by radiologists in 4,099 patients with a history of surgery for breast cancer. All final assessments were classified according to the Breast Imaging Reporting and Data System. The cases with false-negative results were those for which ultrasonography could not identify the cancers during the postoperative follow-up; however, a recurrence was identified by other imaging modalities or palpation within at least 3 months after the negative ultrasonographic results. We analyzed the biopsy recommendation rate, cancer detection rate, and positive predictive value (PPV) for the detection of breast cancers.
Results: Based on 5,343 examinations in 4,099 women, there were 48 recurrent breast cancers with positive US findings (0.9%) and 24 recurrent breast cancers without positive US findings (0.4%). The biopsy recommendation rate was 2.1% per patient and 1.8% per examination. The positive predictive value was 49.5% with 48 breast cancers diagnosed (cancer detection rate, 1.2% per patient and 0.9% per examination).
Conclusion: With a PPV of 49.5%, our results suggest that screening US could be a useful supplementary tool to mammography for the detection of breast cancer in patients after surgery for breast cancer.
